The initial request form belongs on the preceding studio page or its approved form integration. It should ask for first name, email, studio name, studio website or city, and main interest. Marketing consent must be a separate, optional choice.

Optional follow-up details
Once the request flow is connected, optional follow-up questions may cover CCSA membership, screen-printing experience and typical class size. Keep these questions separate from the initial planner request so they do not delay access.
